A London NHS PCT is looking for a Service Manager to head their new Home Rehabilitation Service. The Service is an integrated health and social care team providing care at home for a range of community based patients. The Service Manager will provide leadership for the Rehabilitation Service team and coordinate the continued development of the Service.
